Big Changes Coming to Ohio Division of Liquor Control Forms Soon


Ohio Division of Liquor Control Forms

The Ohio Division of Liquor Control is updating its permit applications forms.


The new forms can be found here.


Of note, there are significant changes in many documents, including:

  • The 4174/4113 Application form is much longer and provides guidance on how to complete the application.

  • More detail is now required for LLC/Corporate Disclosure forms.

  • The "financial verification" worksheet is no longer needed, as the requested information is now requested in the application form, however, supporting documents must still be submitted.

  • Also, applicants who have been open for more than 6 months (rather than 1 year), no longer need to submit financial verification documents.

Although there is no official word yet, the speculation is that Liquor Control plans to switch over to the new forms on July 1, 2022, at which point the old forms will no longer be accepted.


Please note - the new Ohio Division of Liquor Control forms are being updated periodically, so it is highly recommended to continue checking their website to ensure you have the most recent version when applying for a new Ohio liquor permit or transferring an existing Ohio liquor permit.
